Family Caregiver, and Support for Families with Children Investment Hourly Rate: $120/Hour An hourly rate is ideal for achieving specific, short-term goals like completing advance care planning forms or facilitating a meeting with loved ones to talk about your wishes. Journey Packages: Pre-paid bundles of hours for a reduced hourly rate. These packages offer greater value with more regular check-ins and support. Ideal if you want consistent guidance over the coming months and support for your loved ones after your death. With Journey Packages , you don't need to worry about paying invoices each week. 8 hours: $770 16 hours: $1545 24 hours: $2305 |

Caring for a loved one through their end-of-life can be a full time job in terms of your heart-space and time. As a family caregiver, you must navigate health and legal systems, lead difficult conversations, schedule home care and medical appointments, cook meals and chauffeur, all while trying to spend quality time with your loved one and live your own life. Chelsea is an experienced health system navigator with deep knowledge of community services and businesses. I can help you:

Your Holistic Advance Care Plan

These lively and interactive sessions have as many belly laughs as heartfelt tears. More than simply writing your advance medical wishes, our heart-centred, practical process will help you create a plan that cares for your body, mind and spirit. We’ll guide you through the steps to:
You’ll leave with a Holistic Advance Care Plan (instructions for the care you would or would not want if you became incapable of making decisions for yourself), and feeling confident and inspired. Comfort care is an often-forgotten part of end-of-life planning. It can help to remember that one day, we must all live as we are dying. Finding ways to comfort ourselves and loved ones is essential. Using the My Personal Comfort Plan guide and checklist, learn how to write personal comfort wishes that evolve with your particular needs, through your illness and end of life.
